New patients

New registrations

To register with the practice contact reception who will provide you with the necessary forms and practice leaflet. In order that we can establish your current health needs. All newly registered patients must make an appointment with the practice nurses or HCA for a patient health check and medical review, as it normally takes a few weeks for your previous medical records to arrive. You will be assigned a doctor but will be registered with the practice team.

Temporary registrations

We are happy to see any relative or friend staying with you, providing they come from the UK or reciprocating country. Otherwise there will be a charge. They will need to fill in a form prior to being seen.

Fit for work certificates

It is not necessary to have a doctors certificate until you have been absent continuously from work for 7 days including weekend. After 3 days you must obtain a self certificate for your employer. These are available from reception. If you need a private certificate these are chargeable. If you are still unfit after 7 days you will need to see the doctor.

Waiting room

We have a small play area in the waiting rooms which is unsupervised. The new play toys have kindly been donated and we would appreciate parents supervising their children.

Change of personal details

If you change your name, address or contact details (such as your telephone number or e-mail address), please let us know as soon as possible, so that we can update our records. If you are receiving treatment at a hospital it might also be a good idea to inform them of any changes so that both our records are consistent when we communicate with them.
